But for people with Posttraumatic Stress Disorder (PTSD), scary memories can pop up suddenly and feel terrifyingly real. They might avoid certain places or feel constantly on edge. Why does this happen? The answer lies deep inside our heads.
The Science of PTSD Brain Imaging
When someone goes through a highly frightening event, their brain can change how it handles memories and feelings. To understand this better, scientists looked at 34 years of research. They focused specifically on PTSD brain imaging to see exactly what happens inside the brain when it processes trauma. Based on studies sourced from specific medical databases between 1990 and 2024, researchers reviewed functional scans. These are special pictures—like fMRI, PET, and SPECT scans—that show the brain actively working, thinking, and feeling in real-time, rather than just looking at its physical shape. By reviewing these studies, they noticed that a specific network of brain areas behaves differently in people with PTSD.
How It Works: The Brain's Defence System
Think of your brain as a highly trained security team protecting a building. This team includes several important members with long names, like the amygdala, insula, prefrontal cortex, cingulate gyrus, and hippocampus. Together, they form a network that handles memories and emotions. The studies suggest that in people with PTSD, this security team acts differently. Instead of working together in their usual way, the functional scans show altered activity across these specific areas. Because this network mediates how we remember and feel, these functional changes help explain why past memories can suddenly feel like present dangers, or why someone might experience intense hyperarousal and avoidance.
Understanding the Biology of Trauma
This large review of research helps map exactly what happens following trauma. By identifying exactly which brain areas are involved, scientists can better understand the biology of fear and memory. The findings confirm that PTSD is linked to functional changes in how the brain processes emotion and memory. Knowing this helps us see that symptoms like intrusive memories are closely tied to how these brain regions interact.
